Release of Liability Waiver, Fee Schedule, Owner and Animal Information Form

Please click on the blue link provided below, to read over and sign a copy of my release of liability waiver, fee schedule and information pages online prior to your scheduled appointment time. If you have more than one horse please include all of their names if you plan on ever having me out for them too that way you do not need to re-sign. This form is for the 2025 year.

If you are uncomfortable signing online, I will have paper copies on hand during your appointment.  Please note, I, your therapist will NOT do bodywork in any form without the liability waiver being signed. Once signed and submitted/given to me, your therapist, you may at any time request a copy of your signed waiver.
